| FORMS FOR GIVE ME TEN 2008 |
VCOC recommends Chapters tailor the forms to incorporate their own chapter address at the bottom and pre-print the name of chapter (and possibly recruiters name(s). This will make it easier to ensure the chapter receives credit for the recruitment. It may also help you recruit chapter members.
At right are form templates for possible use by Virginia chapters. These Word Documents are each designed to fit on all one page, with no printing needed on back. Insert Your Chapter info before saving and printing.
FIRST, the Free Form (like the yellow MOAA form - with JROTC Instructors added)
SECOND, the Paid Form (Like the MOAA green form - with Life Memberships added.
MOAA PROVIDED Give Me Ten Membership Enrollment Forms in paper copy as part of a kit sent to chapter presidents in January 2008. These MOAA recruiting materials, including forms, are available in PDF Format on MOAA website at this link http://www.moaa.org/givemeten Caution, some of the brochures are generic MOAA publications and not Give Me Ten. The Life Membership brochure, contains a LIfe Membership Form which does not have a place to indicate chapter or recruiter, or that it is a GIve Me Ten recruit. Please use a chapter form, or VCOC template instead.
MOAA ADDRESSES ARE DIFFERENT FOR MAILING FREE VERSUS PAID Give Me Ten forms. Free forms go to Alexandria VA (MOAA provided yellow envelopes). Paid go to Merrifield VA (MOAA provided green envelopes). Again, VCOC recommends using a chapter address on the form or publicity and then the chapter forwards to the appropriate MOAA address. See addresses below.
