At right is a scan of the two Give Me Ten application forms (free and paid) posted as a thumbnail photo 18 January 2010 - both fit on one sheet of 8 1/2 by 11 paper. PDF versions were posted, separately, below April 2010.
Note different forms and mail-to addresses for free versus paid enrollment forms. Also, please be sure forms are completely, legibly filled out, including your chapter name, numerical designation and a recruiter's name (recruiter should be a member of your chapter).
VCOC recommends Chapters tailor the forms to incorporate their own chapter address at the bottom and pre-print the name of chapter (and possibly recruiters name(s). This will make it easier for the chapter to track the numbers recruited and ensure the chapter receives credit from MOAA. It may also help you recruit chapter members, by giving you the addresses of the new GMT members, so you can follow-up with them.
Since no changeable electronic formats have been provided, VCOC recomends that you print out a form (below), add chapter name and cover the address with your chapter address before mass producing. Depending on your chapter's approach, you may want to add the recruiter name as well.
